In partnership with Dementia UK, a dedicated Admiral Nurse Service is available for carers in Brent.
Admiral Nurses are specialist dementia professionals who offer expert guidance, practical advice, and emotional support, helping carers feel confident and empowered in their caring role.
The service consists of two Admiral Nurses who support families and carers of people living with dementia. The carer must be a Brent resident.

- Specialist assessment and advice tailored to carers needs
- Support through challenging transitions
- Guidance on end-of-life care and personalised care planning
- Help navigate systems and services
- Improving dementia awareness and best practice across health and care settings
- Training and support for professionals to raise standards of dementia care
- Safeguarding concerns
If you are supporting someone living with dementia and would like further information, please contact your GP or healthcare professional.
Carers can be referred by their GP or directly via another specialist practitioner including community mental health teams, social prescribers and charity and/or voluntary sector partners.
Referrals should focus on carers who are:
- Experiencing high levels of emotional or psychological distress
- Risk of carer relationship breakdown
- Supporting someone with young onset dementia
- Supporting someone with end-of-life care
