We've answered a few frequently asked questions, but if you have any more, send us an email.
If you're 13 or older, you'll be sent a form and questionnaire to fill in yourself - you can share as much or as little as you like.
We will also ask for background information from your parents, school and GP.
You will then have the chance to talk to one of our clinicians at your appointment, and we want to hear your perspective.
We will also speak with your parent/s to ask about your background health and especially development as a young child.
Once we have reviewed all the information from the forms, questionnaires, interview and clinical observations, we will invite you and your parent to a Feedback clinic to discuss the outcome of the assessment and talk about next steps. We will give you a report which you can share with your school to help with support plans. We will also share your report with the GP to store on your health records.
You'll get a written report in language that makes sense. If you have a diagnosis, it doesn't change who you are - it gives you a language for your experience.

That's completely okay. Talk to someone you trust - a parent, carer, or trusted adult - about your questions and concerns about being assessed. If you and your parent decide that you would not like to be assessed at this stage, please let us know and we will remove you from our waiting list. When you’re ready, a new request for the assessment can be submitted, to SHINE if this still before your 18th birthday, or to the Adult Autism and ADHD services after your 18 birthday.
